29 CFR 1910.147(f)(3)(ii)(D):     Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  (a) Employees working on light installation in the warehouse did not all affix a personal lockout device to isolate the hazardous energy before working on the installation. Only one lock was used to lock out the appropriate breaker for the lights, and the maintenance manager had possession of the key. Employees were exposed to contact with electrical current.

29 CFR 1910.212(a)(2):     Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:  (a) There were two milling drill press machines in the shop area that did not have any guarding affixed to them to protect employees from the caught in hazards associated with these machines. The machine with serial number J-68326 was located in the shop area next to machine #384.

29 CFR 1910.215(a)(2):     Abrasive wheel(s) used on grinding machinery were not provided with safety guard(s) which covered the spindle end, nut, flange projections:  (a) Employees were exposed to caught in hazards from exposed nuts on the abrasive wheel machines # 168, 328, and 207. Proper guarding was not present and the employer therefore did not protect employees from the caught in hazards associated with these machines.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    (a) There were multiple instances of exposed wiring due to improper strain relief. In one instance, insulation was pulled from the fittings of machine #384 BNC - 34T, exposing wiring to the machine and creating the hazard of contact with electrical current for employees working in the area and on the machine.

29 CFR 1910.305(j)(2)(iv):     A receptacle installed in a wet or damp location was not suitable for the location:    (a) The oulet providing electricity to the sump pump near machines 349 and 148 was not properly protected from coming into contact with water being dispensed into the drain near to it. Employees working in the area could come into contact with electrical current and suffer from electrical shock and burns.
