1910.179(j)(2)(iii):  Frequent Inspection: Monthly inspection of overhead crane load hooks, with a certification record which included the date of inspection, the signature of the person who performed the inspection and an identifier of the hook which was inspected, was not performed.   On or about April 17, 2014 at the O & F Machine Products facility- employees in the Blanchard and Honing area of the building were exposed to a struck by hazard from possible failure of load hooks because monthly inspections for deformities or cracks were not accomplished or documented.

29 CFR 1910.212(a)(2)  Machine guarding was not  provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points and rotating parts in locations including, but not limited to:  On or about April 17, 2014 at the O & F Machine Products facility- employees in the main production area were exposed to rotating parts of a Ex-cell-O aluminum/metal lathe that was not complete with any guarding to prevent employee from contact from the rotating parts.   Abatement certification for this item is not required.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:   On or about April 17, 2014 at the O & F Machine Products facility- Employees were exposed to health hazards because the employer failed to provide details of the Hazard Communication Program, specifically information related to safety data sheets.
