29 CFR 1926.452(c)(2): Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would secure vertical members together laterally, and the cross braces were not of such length as would automatically square:    a)  837 Jersey Ave, Jersey City, NJ, - south side parking garage, 7th floor    Crossbracing on the south side of parking garage, 7th floor was not installed on the outside of a tubular frame scaffold to ensure stability.                             Violation observed on or 06/01/2012.    b) 837 Jersey Ave, Jersey City, NJ - south side parking garage, 7th floor      Crossbracing on the south side of parking garage, 7th floor was not installed on the inside of a tubular frame scaffold to ensure stability                              Violation observed on or about 06/05/2012.      c) 837 Jersey Ave, Jersey City, NJ  north side parking garage, 7th floor    Crossbracing on the north side of parking garage, 5th/6th floor was not installed on the outside of a tubular frame scaffold to ensure stability                              Violation observed on or about 06/05/2012.    d) 837 Jersey Ave, Jersey City, NJ  north side parking garage, 7th floor      Crossbracing on the tubular frame scaffold on north side of parking garage, 7th floor was not installed on the entire inside section of the tubular frame scaffold to ensure stability.                               Violation observed on or about 06/06/2012.

Hazards included but were not limited to falls.                        Violation occurred on or about 03/16/2012.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:     a) 837 Jersey Ave, Jersey City, NJ - south side garage, 7th floor    Employees that were walking/working on tubular frame scaffold were not protected from fall hazards of approximately 27 feet.                                    Violation observed on or about 06/01/2012.      Blade Contracting, Inc., was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent 29 CFR 1926.451 (g)(1)(vii) which was contained in OSHA Inspection Number 311441109, Citation Number 1, Item Number 2, with a final order issued on 11/27/2007 with respect to a workplace located at 777  United Nations Plaza, New York, NY.     Blade Contracting, Inc., was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent 29 CFR 1926.451 (g)(1) which was contained in OSHA Inspection Number 313992588, Citation Number 1, Item Number 1, with a final order issued on 07/26/2010 with respect to a workplace located at 465 Washington Blvd, Jersey City, NJ.     Blade Contracting, Inc., was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent 29 CFR 1926.451 (g)(1) which was contained in OSHA Inspection Number 314387010, Citation Number 1, Item Number 2, with a final order issued on 10/13/2010 with respect to a workplace located at 99 Monmouth Street, Red Bank, NJ.

Violation occurred on or about 03/16/2012.      Blade Contracting, Inc., was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent 29 CFR 1926.501 (b)(1) which was contained in OSHA Inspection Number 311058812, Citation Number 1, Item Number 3a, with a final order date issued on 12/21/2007 with respect to a workplace located at 50 Columbus Drive, Jersey City, NJ.
