29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights shall bear on base plates and mud sills or other adequate firm foundation. The employer failed to protect employees from potential fall hazards associated with working off scaffolding that was not effectively supported by base plates and mud sills. The most recent example of this was found on the job site located at 6110 Abbott Dr. Omaha, NE 68110 where the employer had employees on a commercial construction project working off a scaffold with missing base plates and misaligned mud sills, set up on a roof of a structure whereby the roof is covered with river rock; thus failing to provide the scaffold with a stable base without the effective use of base plates and mud sills.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. The employer failed to protect employees from potential fall hazards associated with accessing or egress a scaffold from a position that is more than 2 above or below the access point. The most recent example of this was found on the job site located at 6110 Abbott Dr. Omaha, NE 68110 where the employer had employees on a commercial construction project accessing and egress the scaffold on the roof whereby the lowest access point was greater than 2 above the roof top.

The most recent example of this was found on the job site located at 6110 Abbott Dr. Omaha, NE 68110 where the employer had employees on a commercial construction project working off a scaffold that has not been inspected by a competent person either during erection, modification or prior to use. Scaffold violations include but are not limited to: i. Scaffold platform not fully planked. ii. Improper access and egress in that the first step up from the ground for the integral ladder in the scaffold frame is greater than 2 above the ground. iii. The scaffold is missing base plates and has a base plate that is not fully on the mud sill.

The most recent example of this was found on the job site located at 6110 Abbott Dr. Omaha, NE 68110 where the employer had employees on a commercial construction project working off a scaffold that was erected, disassembled, moved, re-erected and altered all without supervision by a competent person. Scaffold violations include but are not limited to: i. Scaffold platform not fully planked. ii. Improper access and egress in that the first step up from the ground for the integral ladder in the scaffold frame is greater than 2 above the ground. iii. The scaffold is missing base plates and has a base plate that is not fully on the mud sill.
