29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.    At the worksite, on April 9, 2012, employees were reinstalling the roof.  The employees were not protected against falls of approximately 18 feet.

29 CFR 1926.1053(b)(1):     Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder(s) was/were not secured at its top to a rigid support, and/or a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder:    At the worksite, on April 9, 2012, the two ladders used for access and egress of the roof extended approximately 1 foot above the landing surface. This condition exposed employees to falls of approximately 18 feet.
