In completed installations, each outlet box did not have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass did not provide bushings designed for the purpose or did not have smooth, well-rounded surfaces on which the cords may bear.      On or about April 10, 2012, employees were exposed to electric haxards in that the employer that did not ensure that a light swith in the bathroom had a face plate.
