29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. At the site, there were employees installing deck pans from a mobile scaffold without wearing any head protection. The employees on and below the scaffold were at risk of a head injury if a deck pan were to fall.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. At the site, employees were installing deck pans from a mobile scaffold which was erected two bucks high with each buck measuring 5 feet in height. Employees would climb the scaffold's cross bracing in order to gain access to the scaffold's top platform. This method of gaining access exposed the employees to a fall hazard of 12 feet.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. At the site, employees were installing deck pans from a mobile scaffold without a guardrail system installed on the front and sides of the scaffold, thereby exposing the employees to a fall hazard of 12 feet.

29 CFR 1926.452(w)(6)(v): Employees shall not be allowed to ride on scaffolds unless the following conditions exist: No employee is on any part of the scaffold which extends outward beyond the wheels, casters, or other supports. At the site, employees were riding a mobile scaffold from one location to another while on the side bracket platform, thereby exposing an employee that was located on the top platform of the scaffold to a tipping hazard with a fall of 12 feet.
