29 CFR 1910.23(d)(1)(iii): Stairway(s) less than 44 inches wide having both sides open, did not have one stair railing on each side: On or about April 11, 2012 at Rig #1, employer did not ensure that two 5 tread stairways, open on both sides, was equipped with standard railing on each side. Employees were exposed to potential falls from one side of each stairway. a) The stairway was located at the front of the workover rig, leading to rig floor. b) The stairway was located at the rear of the workover rig, leading the rig floor.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Rig #1: On or about April 11, employer did not install machine guarding on two instances: a) The air compressor located on the back side on the workover rig. b)The mud pump motor located on the rear side of the workover rig. These conditions exposed employees to potential caught-between and amputation hazards.
