29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.    (a)  The employer did not have a competent person erect the scaffold, to ensure that it was constructed properly and completely.  This exposed employees to fall hazards form missing guardrails.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:    The employer failed to ensure that employees, conducting masonry work on a fabricated scaffold, were protected from falling by the use of a guard rail system.    (a)  The scaffold was missing guardrails on the ends of the scaffold's 2nd and 3rd levels, exposing employees to a fall hazard.  (b)  The employer did not install a mid-rail on the working levels of the scaffold.    THE HOPWOOD MASONRY, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315121541, CITATION NUMBER1, ITEM NUMBER 2, AND WAS AFFIRMED AS A FINAL ORDER ON JULY 26, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 2730 WEST LISBON AVENUE IN MILWAUKEE, WISCONSIN.
