On or about April 18, 2012, on the south side of the building, employees engaged in soffit installation on an apartment building utilized an extension cord that was hardwired into a panel box exposing employees to electrical hazards.

On or about April 18, 2012, on the south side of the building, employees engaged in soffit installation on an apartment building utilized an extension cord with the missing ground pin.  The employees were exposed to a electrical hazard.

29 CFR 1926.405(g)(2)(iv):  Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws.        On or about April 18, 2012, on the south side of the building, employees engaged in soffit installation on an apartment building utilized a Makita skill saw with the power cord missing a strain relief device exposing employees to an electrical hazard.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.        On or about April 18, 2012, on the south side of the building, an employee engaged in soffit installation on an apartment building worked without fall protection exposing the employee to the hazard of falling approximately 15 feet to the lower level.

On or about April 18, 2012, on the south side of the building, an employee engaged in soffit installation on an apartment building utilized an extension ladder that did not extend 3 feet above the upper landing exposing the employees to the hazard of falling approximately 15 feet to the lower level.
