29 CFR 1910.134(d)(1)(i): Selection of appropriate respirators was not based on the respiratory hazard(s) to which the worker was exposed and user factors that affect respirator performance and reliability:    At the Rig 102 drilling site at Hutchens 1-26H well in Dewey County, Oklahoma, on or about April 12, 2012 and at times prior thereto, the employer provided 3M 5203 Organic Vapor/Acid Gas respirators without particulate filters for employees who were adding 50-pound bags of dry chemicals to mixing hoppers, including, but not limited to, LCM Mica containing respirable crystalline silica and caustic lime powder (Hydrated Lime containing calcium hydroxide and trace amounts crystalline silica).

29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested:    At the Rig 102 drilling site at Hutchens 1-26H well in Dewey County, Oklahoma, on or about April 12, 2012 and at times prior thereto, the employer did not ensure that annual respirator fit testing was provided for employees who were required to wear 3M 5203 Organic Vapor/Acid Gas respirators while adding 50-pound bags of dry chemicals to mixing hoppers, including, but not limited to, LCM Mica containing respirable crystalline silica and caustic lime powder (Hydrated Lime containing calcium hydroxide and trace amounts crystalline silica).

29 CFR 1910.134(g)(1)(i)(A): The employer permitted respirators with tight-fitting facepieces to be worn by employees who had facial hair that came between the sealing surfaces of the facepiece and the face or that interfered with valve function:    At the Rig 102 drilling site at Hutchens 1-26H well in Dewey County, Oklahoma, on or about April 12, 2012 and at times prior thereto, the employer did not ensure that facial hair that interfered with the face-to-facepiece seal of the respirator was not worn by employees who were required to wear 3M 5203 Organic Vapor/Acid Gas respirators while adding bags of dry bulk chemicals to the mud hopper, including, but not limited to, LCM Mica containing respirable crystalline silica and powdered caustic lime (Hydrated Lime).
