Employees were working in an excavation 14 16 feet wide and 7.5 8 feet deep that was not shored, sloped or benched, and the excavation and adjacent areas were not inspected by a competent person.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: Employees installing concrete culvert were exposed to crushing and asphyxiation hazards. The employee in the excavation was approximately 40 feet from a ramp and the employer did not ensure a ladder or other safe means of egress was provided within 25 feet.
