29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables shall not be used:    This violation occurred on April 19, 2012, at a work site located at 7115 Paluxy Drive, Irving, Texas; an employee was exposed to potential electrocution by using a Target cutting saw that had an electrical cord with damaged insulation.    JOSE ALEMAN WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD, OR ITS EQUUIVALENT STANDARD 1926.416(E)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314181801, CITATION 003, ITEM NUMBER 001 AND WAS AFFIRMED AS A FINAL ORDER ON MAY 26, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 324 PALUXY DRIVE, IRVING,TEXAS 75039.
