29 CFR 1910.178(l)(6):     The employer did not certify that each operator has been trained and evaluated as required by paragraph (l) and shall include the name of the operator, the date of training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:  	  The employer did not document the required powered industrial truck training and evaluation for each operator.  There was no written documentation that included the operator's name, date of the training, date of the evaluation and/or the name of the person conducting the evaluation.

29 CFR 1904.29(b)(1):     A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in the detail as required by the regulation.    On or about April 24, 2012 the employer did not provide the required injury or illness descriptions on the OSHA 300 Log for the calendar year 2012.    a.	On or about April 14, 2012, a recordable injury occurred when an employee was loading shingles onto a conveyor. The employee twisted wrong with a bundle of shingle in his/her hands, resulting in a hernia to the groin.  The description in Column F did not describe the object that directly injured the employee.
