29 CFR 1910.67(c)(2)(v):     A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift:    On 04/26/2012, at an establishment located at 6085 Samuell Boulevard, Dallas, Texas; an employee working from an elevated aerial platform when replacing a billboard sign did not wear a body belt (or harness) and lanyard attached to the boom or basket; and was exposed to a fall hazard.

29 CFR 1910.333(c)(3)(iii)(A):     Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) was maintained:    On 04/26/2012, at an establishment located at 6085 Samuel Boulevard, Dallas, Texas; an employee working from an elevated aerial platform when replacing a billboard sign was exposed to an electrical hazard. The platform of the Terex Hi-Ranger aerial device came closer than 10-feet to an energized 7.6 kV overhead power line.

29 CFR 1910.303(b)(2):     Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:      On 04/26/2012, at an establishment located at 6085 Samuell Boulevard, Dallas, Texas; employees using a dual-outlet extension cord when operating a Porter reciprocating saw were exposed to an electrical hazard:     1) The receptacle was designed to be installed in or on a wall and covered by a faceplate.  2) The receptacle was designed to be connected to Romex type non-metallic sheathed cable.
