The employees hand was demonstrated to be at a distance of 2-3 feet from the rotating chuck, as determined on or about 04/18/2012.    (b) Polishing Area: Employees use a Clausing Metal Lathe, no s/n, to polish various pieces of fabricated materials. The employees hand was demonstrated to be at a distance of 6-8 inches from the rotating chuck, as determined on or about 04/18/2012.    (c) Machining Area: Employees use a DoAll, s/n 297-73143, horizontal band saw to cut pipe and angles. The employees were exposed to approximately 12 inches of unused blade, as determined on or about 04/18/2012.    (d) Lab/Testing Area: Employees use a Jet Drill Press, s/n 009828, to drill holes on aluminum plates. The employees hand was demonstrated to be at a distance of 2-3 feet from the unguarded rotating chuck, as determined on or about 04/18/2012.    ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED
