29 CFR 1926.451(g)(1)(vii):     Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.        Worksite:  Employees who were performing chimney work on a work platform on a steep pitch roof were exposed to fall hazards of approximately 20 feet.

29 CFR 1926.1053(b)(1):     Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at the top to a rigid support and a grasping device was not provided.      Worksite:  Employees were using an orange fiberglass step ladder in the closed position in order to gain access to the upper roof level and the side rails did not extend at least three (3) feet above the upper landing surface.
