29 CFR 1910.133(a)(3):     The employer did not ensure that each affected employee who wears prescription lenses while engaged in operations that involve eye hazards wore eye protection that incorporates the prescription in its design, or wore eye protection over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses.    Employees engaged in packing and kitting operations were exposed to potential eye hazards as identified in the employer's personal protective equipment hazard assessment and the employer did not require employees who wore prescriptive lenses to wear eye protection.
