Main Street, Ligonier, PA: On or about April 30, 2012, during roofing work on a roof with an approximate 5 to 6 in 12 pitch (steep pitched roof), the employer did not ensure that fall protection systems/devices were in use where employees were exposed to falls of approximately 14 feet.
