29 CFR 1926.100(a):     Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  (a)(LOCATION)(SPECIFIC OPERATION(S) AND/OR CONDITIONS)        At the job site located on the corner of Delmar and DeBaliviere Ave, in St. Louis MO the employer did not ensure employees wore hard hats while exposed to falling objects.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.1053(b)(15):     Ladder(s) were not inspected by a competent person for visible defects on a periodic basis and/or after an occurrence that could have affected their safe use:  (a) (LOCATION) (IDENTIFY SPECIFIC OPERATIONS(S) AND/OR CONDITIONS)  Note:  See 29 CFR 1926.32(f) for definition of "competent person".        The employer at the job site located at the corner of Delmar and DeBaliviere Ave, St. Louis MO on 10 February 2012, did not ensure the ladder had been inspected for visible defects.  As a result employees used a ladder with a bent rung to enter and exit the trench.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1926.1053(b)(16):     Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.  (a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS(S) AND/OR CONDITIONS)      At the job site located at the corner of Delmar and DeBaliviere Ave, St. Louis MO on 10 February 2012, the employer allowed employees to use a damaged ladder which had a bent rung, to enter and exit the trench.        Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).
