29 CFR 1910.212(a)(1):   One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  The employer does guard all machines with exposed rotating parts to protect the operator and other employees in the machine area.   a. This violation was observed on or about May 1, 2012, when employees at workstation Puma 12S were exposed to caught-in hazards while cleaning metal shavings on and around the unguarded rotating part of the turbo conveyor.  b. This violation was observed on or about May 1, 2012, when employees at workstation Puma 12L were exposed to caught-in hazards while cleaning metal shavings on and around the unguarded rotating part of the turbo conveyor.  c. This violation was observed on or about May 1, 2012, when employees at workstation HBM were exposed to caught-in hazards while cleaning metal shavings on and around the unguarded rotating part of the Enomoto conveyor.

29 CFR 1910.1200(f)(5):   The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged, or marked with the identity and appropriate hazard warnings regarding the chemicals contained therein:  The employer does not ensure that all containers of hazardous chemicals are labeled with the identity and hazard information.  This violation was observed on or about May 1, 2012, in the chemical storage area of the facility when an unlabeled container was being used to store hydraulic oil/and or cutting lubricant.
