29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    On or about May 3, 2012, during construction activities including but not limited to roofing work, the employer did not ensure employees were protected from possible head injuries by ensuring head protection was provided.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502:    On or about May 3, 2012, during construction activities including but not limited to roofing activities on a steep roof, the employer did not ensure employees were protected from fall hazards by ensuring a fall protection system system/device was being used.

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about May 3, 2012, during construction activities including but not limited to roofing activities on a steep roof, the employer did not ensure employees were protected from fall hazards by ensuring the ladder used for access/egress was properly extended above the surface or secured at the top with a grasping device provided.
