29 CFR 1926.352(i): Drums containers, or hollow structures which have contained toxic or flammable substances shall, before welding, cutting, or heating is undertaken on them, either be filled with water or thoroughly cleaned of such substances and ventilated and tested. For welding/heating worked performed on the pvc pipe: On or about April 12, 2012, and at times prior thereto, the employer did not ensure that the pvc pipe being heated to make a bend was filled with water, cleaned, ventilated and tested, exposing the employees working in the area to the hazard of being struck-by flying debrias after the pipe exploded.

29 CFR 1926.352(j): Before heat is applied to a drum, container, or hollow structure, a vent or opening shall be provided for the release of any built-up pressure during the application of heat. For welding/heating work performed on the PVC pipe: On or about April 12, 2012, and at times prior thereto, the employer did not ensure that the pvc pipe being heated to make a bend had a vent or opening to provide for the release of any built-up pressure, exposing the employee to the hazard of being struck by flying debris after the PVC pipe exploded.
