At the time of the inspection the ladder was leaning against the east wall of the trench, near the north end.

Howco Utilities, LLC, was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 314466871, citation number 01, item number 001, and was affirmed as a final order on December 5, 2010, with respect to a workplace located at Cherry St. and E. 18th St. in Kansas City, MO.
