OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushed by and struck-by hazards:      a)   On or about 5/3/12 - Manufacturing area, a Caterpillar forklift, Model MC30, Serial #4HB1196, was operated without a seatbelt.     Among other methods, one feasible and acceptable method to correct this hazard is to ensure that seatbelts are provided and utilized in accordance with ASME B.56.1b-2003, Safety Standard for Low Lift and High Lift Trucks, Section 7.41, Operator Restraint Systems.
