29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     (a) On or about 07/22/14 - Machine shop, the employer did not have a lockout/tagout program for employees engaged in maintenance and servicing of HAAS CNC machines.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) On or about 07/22/14 - Machine shop, the LOBO horizontal band saw did not have the un-used portions of the blade guarded.    (b) On or about 07/22/14 - Machine shop, the Roll-in vertical band saw did not have the un-used portions of the blade guarded.    (c) On or about 07/22/14 - Machine shop, the Victor 1640 horizontal lathe did not have a chuck guard installed.    (d) On or about 07/22/14 - Machine shop, the Sears Craftsman 8 inch table mounted drill press did not have a chuck guard installed.    (e) On or about 07/22/14 - Machine shop, the ACRA milling and drilling machine did not have a chuck guard installed.
