29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:        A)  197 10th Street, Wood-Ridge - NJ: Employees were exposed to fall hazards of approximately 19 feet while using an extension ladder to access a roof. The extension ladder that was used, did not extend at least 3 feet above the landing, on or about March 29th 2012.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)      A)  197 10th Street, Wood-Ridge - NJ: Employees were exposed to fall hazards of approximately 9 and 19 feet while engaged in roofing work on a residential structure without a means of fall protection in place, on or about March 29th 2012.      Chimbaiana Home Improvement was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent 29 CFR 1926.501 (b) (13) which was contained in OSHA inspection # 314677915, Citation 01, Item 001, issued on 07/25/2011 with respect to a workplace located at 112 Orchard Street in Elmwood Park, NJ.
