29 CFR 1910.1030(f)(5): The employer did not obtain or provide the employee with a copy of the evaluating health care professional's written opinion within 15 days of the completion of the evaluation: a) On or about May 2, 2012 and at times prior, the employer did not obtain nor provide to one employee their health care professional's written opinion containing the required elements. A needle stick incident occurred on May 14, 2011. The required elements that were missing are: 1) If the Hepatitis B vaccination was indicated for an employee, or if the employee had received such vaccination. 2) If the employee had been informed of just the evaluation results and if the employee had been told about any medical conditions resulting from exposure to blood or other potentially infectious materials which required further evaluation or treatment. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").
