29 CFR 1910.157(c)(1):  The employer did not provide portable fire extinguishers and did not mount, locate and identify them so that they are readily accessible to employees without subjecting the employees to possible injury.      On March 5, 2012, and at times prior thereto, at FRAC Tech LLC/Odessa in the Iron Shop a portable fire extinguisher located on the north east wall by the bay door was blocked by a large portable air conditioner.  The extinguisher was not readily available for use in the event of a fire.

29 CFR 1910.305(b)(1)(ii):  Unused openings in cabinets, boxes, raceways, equipment cases or housings and fittings were not effectively closed:      On March 5, 2012, and at time prior thereto, at FRAC Tech LLC/Odessa located at 986 S. Maurice, Odessa, Texas 79763.  In the Iron Shop on the east wall a junction box carrying 110 volts was missing a knock out.  Employees were exposed to electric shock.
