29 CFR 1910.178(l)(1)(i):     The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    A)  At the workplace, at the time of inspection the employer did not provide employees with training on the operation of forklifts. The employees operated forklifts daily.

29 CFR 1910.303(g)(1)(ii):     Working space required by this standard may not be used for storage. When normally enclosed live parts are exposed for inspection or servicing, the working space, if in a passageway or general open space, shall be suitably guarded.    A) At the workplace, at the time of inspection, on 5/8/12, access to three electrical panels and two transformers on the west wall of the Binding Area were blocked by a stack of cardboard boxes.

29 CFR 1904.29(a):     Basic requirement. You must use OSHA 300, 300-A, and 301 forms, or equivalent forms, for recordable injuries and illnesses. The OSHA 300 form is called the Log of Work-Related Injuries and Illnesses, the 300-A is the Summary of Work-Related Injuries and Illnesses, and the OSHA 301 form is called the Injury and Illness Incident Report.      A) The employer did not maintain OSHA 300 logs for 2009.    B) The employer did not maintain OSHA 300 logs for 2010.    C) The employer did not maintain OSHA 300 logs for 2011.    D) The employer did not maintain OSHA 300 logs for year to date of 2012.
