29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:      JOB SITE: 936 Evans Avenue, Kirkwood, MO 63122, at this work site, a worker was observed on the top of the residential structure roof, actively installing roof shingles, operating a pneumatic nail gun without any form of eye or face protecton.  The worker was exposed to a struck-by hazard.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:       JOB SITE: 936 Evans Avenue, Kirkwood, MO 63122, at this work site, two workers were observed on top of the porch overhang, actively installing roof shingles, approximately 12 feet 6 inches eave to ground level, and were not protected by guardrail systems, safety net systems, or personal fall arrest systems.  Although the workers were wearing Personal Fall Arrest system, they were not tied down to anchor point.  The workers were exposed to a fall hazard.
