On or about 5/ 7/12, on site, for employees using Cidexplus and Cetylcide II. Both products contain corrosive chemicals, such as but not limited to, sodium carbonate and glutaraldehyde. The employer did not provide an eye wash station.

On or about 5/7/12, on site, for the employees exposed to Cidexplus. Cidexplus contains chemicals such as, but not limited to, glutaraldehyde. The employer did not provide labeling for containers of Cidexplus.

The employer did not mainatin the MSDS for both products.
