29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a.) Deltaville Yachting Center, Mechanical Shop, Sears Bench Grinder, 1/2 HP, Model 39719440, 115/120 volt. On or about May 9, 2012, the tongue guard was approximately 5/8" from the rotating wheel, exceeing the 1/4" requirement, exposing the operator to flying debris should the wheel shatter during operation.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a.) Deltaville Yachting Center, Boatel Building, rear entrance. On or about May 9, 2012 an energized duplex receptacle was missing a coverplate, exposing employees to accidental with live electrical parts. b.) Deltaville Yachting Center, Large Maintenance Building. On or about May 9, 2012, two circuit breaker panels located on the rear wall of the building were not guarded against accidental contact with live electrical parts in that the left panel had two missing breakers, and the right panel had three missing breakers exposing employees to the energized busbars in the rear of the panel.

On or about May 9, 2012, a temporary flexible cord was being used in lieu of permanent wiring in that it was providing electrical service to an overhead fluorescent light fixture.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a.) Shop Area, Northern Drill Press, Model RDM-100F, Serial No., 16771. On or about May 9, 2012, there was no strain relief at the male attachment plug in that the outer insulation of the flexible cord was pulled away from its attachment point.
