29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points: a) On or about May 14, 2012, and at times prior, employees in the vacuum lanes were exposed to electrical hazards where the flexible cords supplying power to the vacuums were not protected from damage caused by vehicles. 29 CFR 1910.305(g)(2)(ii): Flexible cords were repaired in a manner which did not retain the insulation, outer sheath properties, and usage characteristics of the cord being repaired: a) On on or about May 14, 2012, and at times prior, employees in the vacuum lanes were exposed to electrical hazards where flexible cords supplying power to the vacuums had been repaired with electrical tape which did not maintain the integrity of the cords' original insulation.
