29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a.  Front Roof Area of the House: The employees were working installing shingles on the 6 in 12 sloped roof and were exposed to a fall hazard of approximately 9 feet to the ground / concrete below as observed on 5-17-12.    b. Back Side of Roof Area: The employees were not protected from falls of approximately 15 feet while installing shingles on the 4 in 12 and 6 in 12 sloped sections of the rear roof as determined on 5-17-12.      ABATEMENT CERTIFICATION IS REQUIRED FOR THIS ITEM

29 CFR 1926.1053(b)(1):     Where portable ladders were used for access to an upper landing surface and the ladder length did not allows the ladder side rails to extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a. Elevated Platform Built Over the Cab of the Dump Truck: The Werner fiberglas step ladder used to get down from the platform by an employee only extended above the landing approximately 4 inches as observed on 5-17-12.

Elevated Platform Over the Cab of the Dump Truck: The Werner 8 foot fiberglass step ladder used by an employee to get down from the platform which was leaned against the platform in lieu of the legs of the ladder being pulled apart and locked in place as observed on 5-17-12.
