Blake Roofing, LLC., The Open Door, 2630 Kanawha Blvd. Charleston, WV:  On or about 08/17/15, employees working on a steep roof approximately 30' above the ground were not trained in fall protection or the recognition of fall hazards.  ABATEMENT DOCUMENTATION IS REQUIRED.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a.)  Blake Roofing, LLC., The Open Door, 2630 Kanawha Blvd. Charleston WV:  On or about 08/17/15, employees performing roofing work on a 5/12 sloped roof were not protected from falling approximately 30' to the ground.    Blake Roofing, LLC., was previously cited for a violation of the occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 433193, Citation 01, Item number 001 and was affirmed as a final order on 11/06/2012, with respect to a workplace located at 1309 Louden Heights Road, Charleston, WV 25309.    ABATEMENT DOCUMENTATION IS REQUIRED.
