Employees installing shingles with a pneumatic nail gun were exposed to eye injuries, employee were not wearing any type of eye protection while using the nail gun.

Employees working from a 5 in 12 pitch roof were exposed to falls of approximately 16 feet to a lower level. The employer did not initiate and maintain an accident prevention program to control and eliminate the hazards present at the jobsite in accordance with the OSHA regulations.      Calcott Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 313016149, citation number 1, item number 1a and was affirmed as a final order on 8-13-2010, with respect to a workplace located at 608 Copper Line Rd. Maryville, IL 62062.
