29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage.     Employees using an extension cord to power hand tools were exposed to electrical shock; the employer failed to inspect the cord visually before use. The cord had cuts to the outer insulation.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    Employees engaged in residential construction activities were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or any other approved means. Employees were in the process of installing a new roof on an 6 in 12 pitch and 4 in 12 pitch roof. While working on the roof employees was exposed to falls of approximately 10.9 feet to a lower level.    Calcott Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 433331, citation number 2, item number 2 and was affirmed as a final order on 8/24/2012, with respect to a workplace located at 546 Highland Estates Dr. O'Fallon, IL 62269.
