29 CFR 1926.100(a):   Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets:      a   On or about 05/09/2012, Premier Construction Co, Inc., 74 B & 75 Kronprindens Gade, St Thomas VI  Scaffold area:  Employees were moving construction materials to a roof area using a mobile scaffold without wearing protective hard hats to protect them from injury.      Note:  Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.

29 CFR 1926.451(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    a.  On or about 05/09/2012, Premier Construction Co, Inc., 74 B & 75 Kronprindens Gade, St Thomas VI  Scaffold area:  Employees were observed working over a two bed mobile scaffold in which its working platform area was not fully planked.    NOTE:  Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.

29 CFR 1926.451(g)(1):  Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a.  On or about 05/09/2012, Premier Construction Co, Inc., 74 B & 75 Kronprindens Gade, St Thomas VI  Scaffold area:  Employees were observed working over a two bed mobile scaffold 12-6 above the ground level, without a conventional fall protection system in place.    NOTE:   Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.

29 CFR 1926.452(w)(1):   Scaffolds shall be braced by cross, horizontal, or diagonal braces, or combination thereof, to prevent racking or collapse of the scaffold and to secure vertical members together laterally so as to automatically square and align the vertical members. Scaffolds shall be plumb, level, and squared. All brace connections shall be secured.      a.   On or about 05/09/2012, Premier Construction Co, Inc., 74 B & 75 Kronprindens Gade, St Thomas VI  Scaffold area:  Employees were using a two bed tubular frame mobile scaffold to move construction materials to a roof area.  One set of cross braces was missing.    Note:  Certification and documentation of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c) and (d), respectively.
