On or about May 17, 2012, at the intersection of E. 2nd St. and  300 Commerce St., Fort Worth, Texas:  employees were exposed to vehicular traffic and struck-by hazards while engaged in measuring the depth of a man hole and were not adequately trained to recognized and avoid such hazards.

On or about May 17, 2012, at the intersection of E. 2nd St. and 300 Commerce St. Fort Worth, Texas:  Two employees were engaged in taking the depth measurements of a man hole located in the middle of heavy traffic without the use of barricades to control traffic.  The employees were exposed to struck-by hazards.
