29 CFR 1910.219(f)(3):     Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:    In the Laundry building on Dryer #1, employees clean walking/working surfaces behind dryers and make machinery repairs were exposed to being caught-in the unguarded chain and sprocket on the motor which was adjacent to the doorway, approximately three feet above ground level.

29 CFR 1910.303(e)(1)(ii):     Electric equipment shall not be used unless markings have been placed on equipment such as voltage, current, wattage and other ratings as necessary:     At this facility, energized electric panels did not have any identifying markings, employees who inspect, operate and/or touch electrical components are exposed to shock or death by electrocution such as but not limited to the following:    a.  In the Laundry Building on the east wall behind the washers, the electric panel has six circuit breakers and there are no markings indicating the amperage, voltage, manufactures name etc.    b.  In the Maintenance Building on the west wall, there are two electric panels with several circuit breakers without identifying markings for voltage, amperage, etc.

In the Laundry Building on the east wall behind the washers, the electric panel had six circuit breakers and five were unidentified as to their purpose or function.    b.  In the Maintenance Building on the west wall, a smaller electric panel with eight unidentified circuit breakers.    c.   In the Maintenance Building on the west wall, a large electric panel with four unidentified circuit breakers.

29 CFR 1904.29(b)(1):     A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation:    At Lexington Place Nursing & Rehabilitation:  On or about May 17, 2012, the employer did not correctly record the following work-related injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2012:    a.  On or about January 2, 2012, CNA - an employee received a sprain to the wrist and hand and the employer did not enter a check mark in columns J or indicate the number of days in column L, when the case resulted in days of job transfer or restriction.     b.  On or about March 12, 2012, Dietary worker - an employee experienced pain to foot and ankle and the employer did not enter a check mark in columns J or indicate the number of days in column L, when the case resulted in days of job transfer or restriction.
