29 CFR 1926.502(a)(2):  The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection.  On or about May 16, 2012 employees were upgrading equipment on a cell phone tower approximately 104 feet high using a climbing sling wrapped around angle iron for fall protection, exposing employees to a fall hazard.

29 CFR 1926.503(a)(1):  The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about May 16, 2012, employees were upgrading equipment on a cell phone tower approximately 104 feet high by hanging under the work platform and using fall protection that was engaged directly to a rope without receiving fall protection training from the employer, exposing the employees to a fall hazard.
