29 CFR 1926.405(g)(2)(iii):  Flexible cords shall be used only in continuous lengths without splice or tap. Hard service flexible cords No. 12 or larger was repaired and spliced without the splice retaining the insulation, outer sheath properties, and usage characteristics of the cord being spliced.    a)  Rear of Structure:  Employees used an electric Ryobi 6.5 amp reciprocating saw while energized by an extension cord that was spliced in two locations and joined to other flexible cords by means of duct tape; thereby, exposing employees to electrical shock, on or about 3/29/2012.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a)  Rear of Structure:  In order to access a garbage dumpster, employees used a wooden ramp elevated approximately 8' above the ground surface and without means of fall protection; therefore, exposing employees to a fall hazard, on or about 3/29/2012.
