Hard service flexible cords No. 12 or larger was repaired and spliced without the splice retaining the insulation, outer sheath properties, and usage characteristics of the cord being spliced.    a)  Rear of Structure:  Employees used an electric Ryobi 6.5 amp reciprocating saw while energized by an extension cord that was spliced in two locations and joined to other flexible cords by means of duct tape; thereby, exposing employees to electrical shock, on or about 3/29/2012.
