29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section: a) Young Life Crooked Creek Ranch at 3000 County Rd 519, Fraser, CO 80442: On or before May 8, 2012, the employer did not ensure that energy control procedures were in place for cleaning out the local exhaust ventilation (LEV) for the Delta DC 580 planer model 22-451 serial number CKM 00439 in the following instances: 1) The procedures did not include turning off the planer when cleaning out the LEV. An employee swept his hand across the point of operation to clean out wood chips, and experienced an amputation of the tips of the three middle fingers of his left hand; 2) The procedures did not include locking out the circuit breaker; 3) The procedures did not include testing the equipment to verify de-energization. Condition exposed employees to amputation hazards.

Condition exposed employees to amputation hazards. Abatement note: 29 CFR 1910.147(c)(6)(ii) provides that the employer shall certify that the periodic inspections have been performed.
