29 CFR 1926.451(e)(6)(vi): Integrated prefabricated frame scaffold access frames exceeded the maximum spacing between rungs of 16 3/4 inches (43 cm). a) Jobsite, 18 andover Drive, Medford NJ 08055, Front of the residence: An employee of LHJ Construction LLC. was working on a prefabricated frame tubular welded scaffold and using the end frames to access the different levels of the scaffold, the spacing between the rungs measures approximately 19 inches, observed on or about 04/24/2012.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews: a) Jobsite, 18 Andover Drive, Medford NJ 08055, Front of house: An employee LHJConstruction was working on the second buck up, without the use of fall protection, observed on or about 04/24/2012. ABATEMENT NOTE: Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. A standard guardrail system includes a top rail, a midrail, and toe boards. The height of the top rail for scaffolds must be between 38 inches (0.97 m) and 45 inches (1.2 m). When the cross point of cross bracing is used as a top rail, it must be between 38 inches (0.97 m) and 48 inches (1.3 m) above the work platform. Midrails must be installed approximately halfway between the top rail and the platform surface. When a cross point of cross bracing is used as a midrail, it must be between 20 inches (0.5 m) and 30 inches (0.8 m) above the work platform.
