29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered On or about May 16, 2012 at the worksite in the following instances employees were exposed to hazards capable of causing injury. Instance a): Employees working near where limbs were being cut from standing trees before they were felled and exposed to being struck by falling limbs were not using head protection. Instance b): An employee using a chain saw to fell, limb and buck trees was not using head, eye, and face protection was exposed to being struck by wood particles and/or the saw chain should a kick-back occur. Instance c): An employee using a chain saw to fell,limb and buck trees was not using leg protection to prevent being struck by the saw chain. Instance d): Employees working near the limbing and bucking operation and while using the brush chipper were exposed to being struck by flying wood particles and limbs were not using eye and/or face protection.
