29 CFR 1926.501(b)(4)(i):  Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.      Location: Job Site, Roof    a) Employees removing asbestos from a roof next to a skylight (3 feet by 4 feet) were not protected from falling through the skylight to the level below on or about 05/05/12.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.        Location: Job Site, Roof    a) Employees removing asbestos on the job site roof were removing asbestos up to the roof edge parapet without protection from falling, thus exposing employees to falling up to three floors to the ground below on or about 05/05/12.

LOCATION: Job Site, 260 East 143rd Street    On or about 05/05/12, the employer failed to report to OSHA a work-related death within eight hours, thus preventing OSHA from commencing a prompt accident investigation.
