29 CFR 1926.502(d)(16)(iii):     Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:    A. Gutter Boy, Inc., 1213 Peach Street, Erie, PA  16501, roof, east side of the structure, on or about April 26, 2012 and times prior thereto:  An employee installing boots over pipes on the roof of the structure was utilizing a personal fall arrest system with a rope.  The employee was located near the edge of the roof on the northeast side and had slack in the rope exposing him to a free fall.

Gutter Boy, Inc., 1213 Peach Street, Erie, PA  16501, roof, east side of the structure, on or about April 26, 2012 and times prior thereto:  An employee installing boots over pipes on the roof of the structure was utilizing a personal fall arrest system with a rope.  The rope that the employee was utilizing as part of the PFAS was frayed.
