29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. At the site, where the employees were using a flexible cord to supply power to portable saws and hand held drills without the use of a ground fault circuit interrupter or an assured equipment grounding program. This condition exposed the employees to an electrical hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At the site, where the employees pouring and smoothing concrete mixture on the parking garage level of an apartment building that was under construction without the use of adequate fall protection system being installed.
