29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a)  At the worksite, 2140 Lexington, Houston, Texas, on or about October 30, 2012 - Employees were not provided with fall protection training.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a)  At the worksite, 2140 Lexington, Houston, Texas, on or about October 30, 2012 - Two employees were floating concrete.  The employees were not protected against falls greater than 25 feet.    DeMac & Company Concrete Contractors, Inc. was previously cited for a violation of this occupation safety and health standard, which was contained in OSHA inspection number 452095, citation number 1, item number 3, and was affirmed as a final order on September 4, 2012, with respect to a workplace located at 4500 Travis, Houston, Texas, 77002.
